Midtown Application Criteria
1. Applicants must be 18 years or older .
2. Applicants must be homeless and provide a letter from a shelter or a
social service agency
verifying homelessness.
3. Applicants must fully disclose any convictions for criminal or drug
offenses.
Applicants can be rejected based on criminal history or drug offenses, if:
a. Applicant has a history of violent crime within the past year of
applying for assistance.
b. Applicant has a history of drug – related criminal activity within
the past year of
applying for assistance.
An exception is, if the applicant can demonstrate that he or she:
a. Has an addiction to a controlled substance, has a record of such
impairment or is regarded as having such an impairment:
and
b. Has recovered from such addiction and does not currently use or
possess controlled substances.
All applicants with an admitted history may be required to provide a
recommendation from a parole officer, probation officer or a caseworker.
Applicants may be required to provide documentation of completion of a
rehabilitation program.
4. Applicants must be willing to fully complete the application process and must
provide the following documentation:
1. Photo I.D. (Driver’s License, State ID or DD 214)
2. Birth Certificate
3. Social Security Card
4. Verification of homelessness (shelter letter or an Agency Reference
Letter)
5. Verification of income ( i.e., ssi, pay stub) (when
applicable)
6. Documentation showing completion of a Drug Rehabilitation Program
(when applicable)
