Cleveland Transition Center (Reentry)

The Cleveland Transition Center is a residential, pilot program developed to meet the needs of Cleveland residents who are returning home from incarceration. The Cleveland Transition Center offers men and women the opportunity to develop new life skill strategies to better prepare them for community reintegration.


To assist with reentry, clients are provided with a range of services including case management, cognitive therapies, mental health services, substance abuse treatment, employment and housing assistance, parenting skills enhancement, and family support services. A daily regimen of structured activities helps clients develop a sense of responsibility and self-sufficiency. While in residency, clients can receive visitors at the facility. Because one of the primary focuses of the program is to reduce homelessness, assistance with housing is a key component of the clients' release plans; and rental deposit assistance is an incentive for those who successfully complete the program.


Cleveland residents who are returning to the community upon either judicial release from prison (early release granted by sentencing authority) or straight release (released from incarceration without any supervision requirements), or other mandatory participation requirement are eligible.


The Cleveland Transition Center is also available to previously incarcerated people who have been released within the last six months and are currently homeless. For eligibility and referral information, an intake screener can be contacted by calling (216) 881-7882.


The Cleveland Transition Center was developed through a partnership between the City of Cleveland, the Ohio Department of Rehabilitation and Correction, and Oriana House, Inc.
